If you google "female android modeling in maya", it looks like someone posted DT's old DVD tutorial up on youtube. I believe Part 3 or 4 goes over modeling the shoe/boot using poly and fine-tuning as sub-d.
Josh Robinson has a 21-page boot box modeling tutorial in PDF format that you could try. Sorry its taken me so long to get around to this, but here is a video to answer your initial question - which was to create a NURBs sandal shape based on a Rhino 3D tutorial.
For the last question about full boots and details like folds and creases I'd suggest starting with the you tube video that nitro suggested. Okay, as promised here is another video (bit longer then I wanted it to be) that covers a basic polygon modeling approach to modeling the sandal from the last NURBs based tutorial.
There are many ways to approach this. The box modeling method is covered someplace on you tube, as nitro mentioned. So I will just briefly touch on it here and leave it to you to find the full version. Instead, I will demonstrate another method, by first using create polygon and edge extrusion to create the basic outline of the foot as a planar polygonal surface.. Then copy this and shape it to form the top and bottom the sandal, and then use bridge, fill hole, extrude, insert edge loop, and beveling to create the side of the sandal and tighten up the edges for smoothing.
