As a long shot before reinstalling and because this happened to me once, once you have hit the extrude face button press the move key (ie W) again. The manipulator may then appear.

I experienced a short time when my manipulators didn't automatically come on when I hit extrude face or clicked on objects, despite being in the right mode. I reset preferences in the end and that cured it.
