go to display/ heads up display/ poly count. where the mesh seems to split there was probably multiple points, just select the points and if theres 2 or more points ontop of eachother just goto edit polys/ merge verticles. when i have this problem i just select every vert and just merge verticles and set a limit of 0.0001
