How to create this structure

Can someone tell me how to begin in order to make a disc that has two holes like in this sketch, please?
I know how to make a single-holed disc, but I need a second, smaller hole between the center and the edge of the circle.
Things that I was thinking of using: tube or thorus, but maybe I am looking at the wrong objects to start with. Unless it is possible to just draw it with the slice tool, but that might just ruin the smoothness of the disc's surface...

Thanks but how/where do I find it and use it? I've never used it before. And what is that object?
EDIT: Disregard that, I found a vid about where to find it. So basically drill a hole in it with a cylinder I guess.

After using the boolean tool you do need to clean the mesh up afterwards..............dave

How and what does that imply?
I've used the boolean on this with difference (though did not smooth the disc). So far I am happy with the way it looks.

It`s good practice to make your model all quads if possible leaving ngon(polys with more then 4 sides) is a no no in the 3d industry as a whole(exeption some game engines), Thats why some poeple dont like booleans as they can leave a lot of cleaning upuser added image...............dave

Ah so that's how a hexagonal hole can be turned to look smooth when in HQ mode. Thanks. Well, not sure if this one can be normally cleaned up while still looking smooth. Not sure if I want to, seeing that it looks like this, lol:

Whoa, tried to use Booleans difference with a disc like yours and a hexagonal prism... and both just disappear. Why?

Yes this some times happens there is normally some thing wrong with the mesh.............dave

It's odd, when I used it on that more complex disc (which is a pipe with 50 or so rays), it made a perfectly round hole.
That disc also has a few more divisions on the height.
Oh well, for now the current disc seems to do well for a clock dial.

I wonder if boolean difference will work for making lunar phases out of discs.
