How do you extrude like this guy did?

Hey guys,

Well the title explains it all. The screenshots are below. Basically I'm trying to make a hand and this guy did it in a way that doesn't make much sense to me... anyone noe a solution?

He has unticked keep faces together in the edit mesh menu................dave

Wait but how does he create a new set of smaller faces? In pic 2

After extruding with keep faces together unchecked then scale the extruded faces inwards with the scale tool.
