clothing questions

One of the things I'm messing with is Cloth, trying to figure out what I need to do in maya to get the results I want.
But I'm having some problems.

1) On a shirt I made, although the front form-fits nicely, the back and top of the garmet stay the equivalent of 1-2 inches away from the figure (in simulation mode, with offset at about .075).

2) How are you supposed to do a skirt?

3) Does anyone know of a way to make a single garmet tight in some areas, but loose in others? like a shirt with poofy sleeves and tight cuffs, or pantaloons?

any help would be appreciated - I'm trying to figure things out...

Will post WIP when I have something to post, along these lines.
