New Polygons start with rotated scale axis

When you create a polygon, it should start fresh. Perfect dimensions, subdivisions, and axis. Everyone I make the axis is already skewed, so when I scale it, it does not scale the way I like. The axis for rotation and translating are perfect. I've tried deleting the history, centering the pivot, reverting all settings to default, and restarting Maya.

Try resetting the tool settings for the scale manipulator. The scale axis is defined by the tool, not the geometry. It's possible you have it set to a custom axis.
